I have that exact item only the total length is about 3 inches so it ,must have been a machinist adjustable wrench for square nuts is best I can guess.

What ever you call it, the primary use of the smaller ones was on the old English bicycles – think of that small bag hanging from seat of the old English racers – this was an essential tool in that kit.
