That might count as fail safe on a safety critical item if the nut falls off. But makes servicing a pain….
Done from the factory during build – the assembly worker needs to be QC’d to make sure this doesn’t happen. Not that they care, mind you.
Most of the time it’s not a safety feature but a bolt that was installed on a complete assembly which ends up being captured after it’s installed.
Or it’s trapped by an assembly that was added after the bolt was put in.
You can tear it completely apart, or… Cut it off and put a new one in the other way; as indicated above.
